Consents and Forms

Parents/Carers must complete and sign a parental agreement for us to administer medicine should the child require to have medication during school hours.

School admissions applications are submitted directly through Waltham Forest. Once a pupil is allocated a place at Salisbury Manor Primary School, parents/carers must complete our application form before their child starts.

GDPR Consent - Direct Marketing Consent

Your child’s safety is very important to us here at Salisbury Manor Primary School.  We recognise that as some children reach Year 5 and 6, they may feel more confident and ready to travel home on their own, with your permission. If you would like to give your child permission to walk home alone, a Home Walker Consent Form must be completed. This can be done by filling out the form below or by providing consent through your Arbor account.

Holidays during term time are not encouraged. However, if families need to travel during this period, a Leave of Absence Request form must be completed.

All parents must complete consent forms for their child, including permissions for photography, animal handling, and food handling. Please find our consent form below. If you wish to make any updates, you can either fill out the form , contact the school office via email or make changes on yoru Arbor account.
